Our Referral Policy

Our referrals are received over a secure fax server and are scheduled in the order in which they are received.

**Please note that we do not see patients under the age of 18. ALL referral packets must include the following information:

  • Patient Information Sheet
  • Copy of CURRENT insurance card(s)
  • Last (or relevant) office note
  • Relevant labs and/or radiology
  • Insurance authorization, if needed

Important update regarding our gastroenterology services at Lexington Medical Center

Effective June 25th, 2026, Consultants in Gastroenterology will no longer provide inpatient gastroenterology services at Lexington Medical Center. We are grateful for the time we spent collaborating, and our core mission remains unchanged.

With this, we are excited to announce our new Urgent Care Access Program to ensure our community receives uninterrupted care.

Office Update
